Representation in SGA
Once your club/organization has been recognized by Penn State Wilkes-Barre (Student Affairs Office) you can request to have your voices heard in the PSUWB SGA. Fill out this Request for Representation, and email a copy of your organization’s constitution to the SGA Secretary. It will be discussed the at the following SGA Meeting, and your status will be made known within 7-10 days.

Absentee Excuse Form This form must be completed no less than 30 min prior to the meeting in which you are requesting to be excused.
Club Exec Board & Members Roster Form Executive board members must submit their name, email, and club meeting day/time on a semester by semester basis. An updated list of club/org members is to be submitted each semester. New members can be added by re-submitting the Executive Board and Membership Roster form during the academic year.

Please be sure to register your Community Service events with the Student Activities Office at least 10-14 days prior to the event to discuss procedures and protocol of agencies in order to maintain your SGA representation eligibility, as detailed in the Student Government Association Constitution.
Please fill out the Activity Intent Form at least 10-14 days prior to your intended date. Watch your Office 365 psu.edu email to schedule a meeting with the Student Activities Office to discuss your ideas and for guidance on University Policies, Procedures, and Risk Management Guidelines. All events will be approved on a case by case basis. *Only approved events can seek out funding from Student Activity Fee, SGA Allocations, or a PSU Department.*

Meal Expense Form This form is to be completed if your club/org had a meal as part of your travel, event, activity. Please remember we are TAX EXEMPT; you must tell the wait staff prior to ordering and check your bill before paying.

Allocation Request Form Use this form if requesting funds from your SGA. This form should be sent to SGA Treasurer Jay Shemanski for approval.
Purchase Order Request Complete a purchase order when making approved purchases.
Gas Reimbursement Calculator Gas is reimbursed based on the calculated formula found on this form.
Fundraising
All fundraising events must be registered with the Student Activities Office. The first step in requesting a fundraiser is completing the Activity Intent Form. Fundraising ideas will be reviewed and approved using the Risk Management guidelines and the non-competition ideology. The form must be submitted no later than 14 days prior to the beginning of the proposed fundraiser. If clubs or individual members of clubs are not following Penn State’s policies and rules, the event can be shut down immediately. The PSUWB SGA purchasing card should be used for purchases whenever possible. This card can be signed out in the Student Affairs Office.
